Celebrating Doctor’s Day: Thank You to Our Primary Care Physicians

Today, on Doctor’s Day, we want to say a big thank you to the amazing doctors who work with the Haldimand Family Health Team (HFHT). These doctors care for patients across our community and play a very important role in helping people stay healthy.

Working Together: Doctors and Health Team Members

At HFHT, we believe the best care happens when healthcare providers work as a team. Our doctors work closely with other health professionals including nurse practitioners, registered nurses, dietitians, social workers, kinesiologists, and respiratory educators. We refer to these team members as Interprofessional Health Providers, or IHPs.

Doctors (and Nurse Practitioners) diagnose and treat medical issues, while the IHPs provide support in other areas like nutrition, mental health, exercise, and managing long-term health conditions. By working together, we make sure patients get the full support they need—not just for one problem, but for their overall well-being.

Our Affiliated Physicians

Our affiliated doctors work in Family Health Organizations (FHOs) across Haldimand County. In this team-based approach, doctors support one another, so even if your doctor isn’t available, another doctor from the team can step in to help. This means patients are more likely to get care when they need it.

When you are a patient of an affiliated physician, you also have access to all the HFHT programs and services. This includes things like health education, disease prevention, and special programs that support physical and mental health.
