The Haldimand FHT is a group of interdisciplinary healthcare professionals, including Nurse Practitioners, Registered Nurses, Dietitians, Social Workers, Kinesiologist, Respiratory Educators and support staff, that provide comprehensive team-based care.

Services Available to All Members of the Community
The Haldimand Family Health Team is able to offer select services to any residents of Haldimand County. A family health care provider is not required to access these services. Please contact one of our clinics if you would like to book an appointment.
Sexual Health Appointments
Our Nurse Practitioners and Registered Nurses provide birth control consultations, STI testing and treatment, and HPV vaccines.
Cancer Screening Services
Residents can access breast, cervical, and colorectal cancer screenings with our team, including assessments and pap tests.
Immunizations
We offer publicly funded immunizations for all ages, including COVID and flu shots. Appointments can be scheduled in our clinics.
Virtual Group Education Sessions
A variety of virtual group sessions are available covering multiple health, wellness and chronic disease topics.
