Our Services

We offer a variety of services, some of which require participants to be patients of our affiliated physicians and some of which are open to all members of the community in Haldimand.

Services Available to All Members of the Community

The Haldimand Family Health Team is able to offer select services to any residents of Haldimand County. A family health care provider is not required to access these services. If you would like to book an appointment, please call one of our clinics.

Our Nurse Practitioners and Registered Nurses meet with you to discuss birth control options, sexually transmitted infection testing and treatment, and prescribe and administer the HPV vaccine. Our Nurse Practitioners can provide IUD and Nexplanon insertions at our Dunnville clinic.

For residents of Haldimand Country (with or without a primary care physician) who are eligible for breast, cervical and colorectal cancer screening, appointments can be made with our Nurse Practitioners and Nurses to assess, order appropriate testing or complete pap tests.

The Family Health Team will provide all publicly funded immunizations to those of all ages. Appointments can be made by calling our clinics. During the fall this also includes COVID immunizations and flu shots.

A variety of virtual group sessions are available covering multiple health, wellness and chronic disease topics.

Services for People With a Haldimand FHT Affiliated Physician

The Haldimand Family Health Team offers a variety of services for people who are patients of our affiliated physicians. These services focus on health promotion, chronic disease prevention, and providing people with education and support on a variety of health concerns. If you are interested in booking an appointment for any of our services, please call one of our clinics.

For all services, patients can self-refer or be referred by your physician.

A Health Prevention appointment is booked with a Registered Nurse and takes the place of the “yearly physical”. The nurse will take your blood pressure, height, weight and review immunizations. The nurse may also order updated blood tests, routine cancer screening or bone mineral density tests as needed. For women, our nurses may also provide a pap test if required.

Well Baby appointments are booked with a Registered Nurse. The nurse will complete a Well Baby check that includes height, weight, growth and review the baby’s growth progression. The nurse will review, discuss and provide any immunizations that are required. The nurses are also a support to answer any questions parents may have.

Virtual Introduction to Solids and Picky Eating classes, run by our Registered Dietitians, are also available to assist parents in developing and strategizing healthy eating habits.

Please chat with our nurses and about all of the services that are available as part of the Well Baby services.

Our perinatal (pre-birth) service is led by our Registered Nurses. The nurse can provide a pregnancy test and confirmation, order needed blood tests and ultrasounds and discuss options for future prenatal care. We will offer regular checks-in with the nurse to complement your appointments with your Obstetrician.

After the delivery of your baby, the nurses will continue to check in with you when you come for Well Baby visits.

As needed, appointments can be made with our Kinesiologist to discuss pelvic health postpartum.

Our Certified Respiratory Educators (CRE) are available to meet with you once or on a regular basis to help you manage your Chronic Obstructive Pulmonary (lung) Disease (COPD). COPD includes chronic lung diseases such as Emphysema and Congestive Heart Failure. We will help you review medications, discuss how to use medications, answer any questions about COPD, and help you to develop an action plan for flare ups.

Patients can also book with our Kinesiologist who can assess and provide exercises to improve endurance and help you manage day-to-day activities.

Smoking cessation counseling is offered by our Certified Respiratory Educators. They will review the options available to you to help you quit smoking and help you to set personalized SMART goals. We offer a nicotine replacement (STOP) that is sponsored through the Centre for Addictions and Mental Health (CAMH).

The Diabetes Program is run by our dietitians and nurses. They meet with you to provide support and education on diabetes management, nutrition, and proper foot care. They will also explain the medications that you may be using to manage your diabetes and help you set SMART goals to improve your diabetes.

Our dietitians are available to meet with you to discuss a variety of nutrition topics or medical concerns and how nutrition can impact your health. Topics we discuss include:

  • gastrointestinal issues
  • food intolerances
  • heart health
  • general healthy eating
  • weight gain or loss
  • malnutrition
  • disordered eating

We provide assessment, education and support for:

  • healthy aging
  • osteoporosis
  • fall prevention
  • arthritis
  • aches and pains in pregnancy
  • sprains and strains
  • joint or bone injuries
  • exercise to help manage chronic conditions like diabetes, depression, and hypertension

The Mental Health Counselling Program is initiated by self-referral. All individuals meet with our Systems Navigator first to have a brief discussion about your needs. Our goal is to set you up with the most appropriate services available. Some services may be within the Family Health Team, while others may be additional services that we will help you access.

Haldimand FHT’s counselling program is centred around 5-6 solution-based counselling sessions with a Registered Social Worker. The program focuses on those struggling with:

  • mild to moderate anxiety
  • grief
  • mild to moderate depression
  • sleep
  • emotional dysregulation
  • relationship issues
  • adjustment related to life stressors (job loss, empty nester)

Led by one of our Nurse Practitioners, these appointments are designed to provide medical and psychosocial gender affirming care to gender diverse individuals in a safe and respectful manner.
