The Haldimand Family Health Team (FHT) was created in 2006 as a means to provide team-based care to patients within the Haldimand area. The Haldimand FHT started small, supporting a single group of nine physicians in Dunnville and Hagersville with a Registered Nurse, Nurse Practitioner and a Social Worker. In 2019, the Haldimand FHT expanded to support 80% of the primary care physicians in Haldimand across Dunnville, Hagersville and Caledonia. Staffing expanded in 2019 and again in late 2021.
The Haldimand FHT oversees an interdisciplinary team of Nurse Practitioners, Registered Nurses, Registered Dietitians, Registered Social Workers, Registered Kinesiologist, System Navigator, Certified Respiratory Educator, Registered Respiratory Therapist and administrative staff to provide team-based care to our many affiliated physicians.
The Haldimand FHT is governed by a dedicated Board of Directors that is made up of a diverse group of committed and skilled community members and physicians. The Haldimand FHT is part of the Greater Hamilton Health Network (GHHN) Ontario Health Team (OHT) and GHHN Primary Care Network (PCN). We strive to provide comprehensive primary care to all residents in Haldimand County.

Land Acknowledgement
We humbly acknowledge that the land on which we provide services is located on the ancestral lands of many generations of First Nations peoples. With gratitude, we recognize that the settler population has been able to benefit from this beautiful land.
