The Haldimand Family Health Team (FHT) has numerous partners and collaborators. Funded by the Ministry of Health through Ontario Health, the Haldimand FHT is part of a larger collective of health and social services through the Greater Hamilton Health Network (GHHN) Ontario Health Team (OHT).
Greater Hamilton Health Network (GHHN) Ontario Health Team (OHT)
The Greater Hamilton Health Network (GHHN) is one of the first Ontario Health Teams across the province, with a purpose of transforming healthcare in partnership with patients, families, care partners, primary care, local organizations and the community. A collaboration of local health and social service partners, the GHHN includes representation from more than 50 organizations. Funded by the Government of Ontario, the GHHN is a dedicated group of professionals, organizations and people with lived experience working to co-design a patient centred, health system grounded in engagement, health equity and the local needs of the communities we serve across Hamilton, Haldimand, and Niagara Northwest.
Haldimand is represented by many Haldimand organizations including the Haldimand Family Health Team at many GHHN governance and service planning tables including the GHHN’s Board of Directors, Primary Care Network (PCN), Haldimand Stakeholder Council, and other Stakeholder Councils including Mental Health and Addiction, Primary Care, Partnership and Digital Health. The patient, caregiver and lived experience voice is also represented through Haldimand Patient Advisors on the Patient Family Care Partner Leadership Network.
